Year 7 Vaccinations

Dear Parents/Guardians,

If your child is in Year 7 in 2025, if not done already, please provide online consent for school vaccinations. Vaccination will occur 24 July, 2025, Week1, Term 3.

Log in using your ServiceNSW account, or create an account.

Complete the School Vaccination Consent Form.

No Medicare Card? Request a form from the school.

Remember, your child will only be vaccinated at school if we receive your consent.
